Physician and Medical Release Form Notice
Your child's registration is not complete until we have received clearance from your child's physician. Each student is required to submit a completed Physician's Release Form for each sport they play. This does not apply to the "Little" sports.
AMA students must submit a completed Physician's Release Form to the school office in an envelope labeled AMAAA with the student's name, grade, and sport registered for.

The Pittsburgh Catholic Angels Field Hockey team is open to girls registered at any Diocese school or local Catholic school. This team will play against community teams, clubs, and private schools. The program is run by PGH Field Hockey, LLC.

Team Parent Volunteers Needed!
An AMA student's involvement in athletics requires parents or guardians to volunteer. Volunteering includes working at home games, collecting entrance fees, running concessions, selling raffles, etc.
Each team at the school also must have a volunteer to serve as Team Parent/Manager to coordinate all volunteer activities for the team.
All volunteers must have clearances. To volunteer within the Pittsburgh Catholic Diocese, in any function, all adults over the age of 18 must have up-to-date clearances through the diocese. Sign in to the Diocesan database (VIRTUS) at https://diopitt.org/school-volunteers to check your status or begin this process.
